Workers Comp Losses (Employees)

ALL work injuries, no matter how slight, MUST be reported to Brotherhood Mutual.

Procedure:

• Fill out the Workers Compensation - First Report of Injury or Illness form as completely as possible within 24 hours of the injury
• Fax the form directly to Brotherhood Mutual at 260.482.3589
• Also fax the form to Select Church Insurance Services at 630.379.0122
